- Plan of care
Patients in the Population Health program are assigned a dedicated team. If you’re admitted to hospital, this team develops a plan of care during your stay that addresses your specific needs. Additionally, they work with staff and providers across the system to make sure that you receive the care you need not only while in-hospital but also after you are discharged.
Following your hospital stay, we reach out to you by phone to provide more information on our services and to follow up on any needs you may have. - Home Health Services
Home health service is a short-term solution for patients who need to remain in the home to receive treatment until recovery. The goal of Home Health is to teach and train the patient, family or caregiver on how to manage the condition competently and comfortably. - Virtual Care via Telehealth
The Population Health team offers an alternative to in-person appointments via telehealth using live video chat. It’s a convenient way to get care after-hours, when you’re too sick or busy to see someone in person, or even when you are traveling. - Remote Patient Monitoring
Memorial Healthcare System’s remote monitoring program provides an opportunity for early intervention. We use various devices, dependent on the patient’s condition, to track important health information. Devices monitor key health measurements then automatically send your results to your healthcare provider allowing them to recommend more timely treatments. The result is prevention of serious health complications and reduced hospital visits.
The Population Health team assists patients, who consent to receiving help, with information about resources such as:
- Transportation
- Medical Equipment
- Food
- Housing
In partnership with our Community Services Social Determinants of Health HUB we’re able to facilitate connections and reduce confusion for patients trying to access resources. Ultimately, our goal is to empower patients to advocate for themselves (teach them to fish), so that they’re better able to help themselves when they leave our facilities.
Population Health Management was a great help- there was someone there if I needed them as I recovered. I live alone and have several health issues that I have been dealing with for some time and it was refreshing to have someone to talk to if I needed and who understood if I called to ask a question. This is a great service.
